This is our free support service for people with two or more long-term health conditions or a disability. We can meet you where you feel comfortable, be it on the phone, at our office or we can come to you at home or whatever is best for you..

HĀ ORA

Hā Ora is not a stand-alone project. It is embedded within Te Ora Puāwai’s integrated support model, which includes:

Waiora – Clinical wellness centre (nurses, doctors, and allied health).
Māia – Navigation, advocacy, and mediation for people with long-term conditions.
Whānau Hauora – Resource and activation fund for health and wellbeing.
Te Piringa – Housing challenge support for disabled whānau.
Mana Tautoko – Kaumātua/kuia purpose and practical support network.

Through these connections, Hā Ora functions as a whole-of-community engagement and referral hub, creating end-to-end pathways from initial contact to sustained support.
