This is our free support service for people with two or more long-term health conditions or a disability. We can meet you where you feel comfortable, be it on the phone, at our office, or we can come to you at home, or whatever is best for you.

HĀ ORA

Hā Ora is not a stand-alone project. It is embedded within Te Ora Puāwai’s integrated support model, which includes:

• Waiora – Clinical wellness centre (nurses, doctors, and allied health).

• Māia – Navigation, advocacy, and mediation for people with long-term conditions.

• Whānau Hauora – Resource and activation fund for health and wellbeing.

• Te Piringa – Housing challenge support for disabled whānau.

• Mana Tautoko – Kaumātua/kuia purpose and practical support network.

Through these connections, Hā Ora functions as a whole-of-community engagement and referral hub, creating end-to-end pathways from initial contact to sustained support.

Our team strive to communicate better with you to understand your needs and priorities, help to coordinate with providers, and support you in navigating the services and systems to help you. Whether it's advice or advocacy we'll plan and work with you to meet your needs for free.

WAIORA WELLNESS CENTRE

Waiora Wellness Centre is a new local GP and primary care service from Te Ora Puāwai in Mangere/Airport Oaks.
The centre provides clinical and social wraparound services, and we can meet face-to-face or come to you.
Founding whanau enrolments are open!
Enrol now and be one of the first 500 to join.  
If you’re one of the first 500 enrolled, you’ll receive:

• A free whanau health planning session

• One free GP or NP consultation

SANEOS

Do you have a wound that isn’t healing, keeps returning, looks infected, or is causing you concern?

SANEOS is here to help.

We provide timely and accessible wound-care assessment and support for:
✅ Slow-healing or recurring wounds
✅ Diabetic foot wounds and ulcers
✅ Leg ulcers and circulation-related wounds
✅ Pressure injuries or bedsores
✅ Surgical wounds needing ongoing care
✅ Skin tears, cuts and grazes
✅ Infected or concerning wounds
✅ Anyone unsure about the care their wound needs
 

• Self-referrals welcome

• GPs, organisations, whānau and friends can also refer
